How to create staff activity records?

Staffs have been assigned to manage the co-curriculum activities, eg to be teacher in charge for Kelab Bahasa Malaysia. User is required to use Staff Activity option to input the assigned duties for managing not only for co-curriculum activities but also for others eg PIBG.
